Life Care Plan
A Life Care Plan is a tool used to estimate medical and non-medical needs and associated costs of a person with catastrophic injuries. In collaboration with health care providers, vendors, patient, and family, a long term plan is created to address the needs of the persona as a whole. Medical supplies, treatments, medical care, transportation, mobility, and physical needs are some of the many areas that are addressed in the Plan.
Review of medical records, patient assessments, and interviews with treating providers are conducted to develop an individualized plan that promotes independence and quality of life.
Medical care and costs are estimates based on the assessment and may be revised as life situation changes occur.
Major types of injuries include, but are not limited to:
- Spinal Cord Injury
- Traumatic Brain Injury
- Burns and Amputations
- Cerebral Palsy
- Chronic Pain
- Multiple Traumas
- Chronic Health Conditions
